Lifestyle has a huge impact on most chronic diseases, and therefore has huge potential in improving (or reversing) chronic disease. Chronic diseases require specialised dietary management, and we know how difficult and confusing it can be to try when managing two or more conditions at once.

Your treating GP can refer you for chronic disease management under your Enhanced Primary Care (EPC) plan for individualised nutritional assessment, education and management.

Currently all consultations provided under a valid EPC referral are bulk billed! (Note – there is a one-off new patient fee of $20)

Veterans can also access these services through a separate DVA referral – ask your doctor for further details about D904 referrals.
